Encounter Form – The NNCC Encounter Form was designed by a cross-functional team of clinicians, operations managers, billing managers, and data specialists participating in the NNCC data network. The form was designed to improve data collection by including a larger number of CPT and diagnosis codes as well as additional data elements of interest.
